The NDIS stands for the National Disability Insurance Scheme. If you would like to apply for the NDIS, visit the am I eligible checklist on the NDIS website. To meet the eligibility criteria, you must:
- be aged under 65
- be an Australian citizen, permanent Visa holder, or Protected Special Category Visa holder
- need support from another person because of a permanent and significant disability
- use equipment because of a permanent and significant disability
- need support now to reduce your future needs

If you have been receiving disability support services, the National Disability Insurance Agency will notify you when you can apply for the NDIS.
I’m eligible, how do I apply for the NDIS?
If you meet the eligibility criteria, you will need to do the following to apply for the NDIS:
- make an Access Request
- provide information about your disability
What is an Access Request?
Complete an Access Request form to become a participant in the NDIS.
You can make an Access Request in two ways:
- Verbal Access Request
- Access Request Form
You can get an Access Request form from your:
- Local Area Coordinator
- early Childhood Early Intervention partner (if your child is under 7)
- local NDIA office
- by telephoning 1800 800 110
Providing information on disability
The NDIA will ask you to provide information on your disability once you make your Access Request. The NDIA uses this information to determine who is eligible for the NDIS.
You will also need to work with your treating health professional to provide information on your disability.
